Admission Policy & Procedure
Butterflyz LLC provides placement for clients facing developmental disabilities, substance abuse issues, behavioral and emotional challenges, educational needs, and youth preparing for independent living. Placements are made in accordance with Wisconsin Children’s Code 48.63 and HFS 57 (Group Home Foster Care for Children).
Procedure
Referring agencies must provide background information to assess the suitability of each placement, including details on age, developmental level, legal history, education, behavioral concerns, and required supervision. Supporting documents—such as treatment and permanency plans, court orders, psychiatric evaluations, and medical records—will also be required.
Prior to placement, caretakers and social work staff will review potential residents, ensuring the safety and well-being of current group home clients remains the top priority. Pre-placement visits can be arranged to determine suitability. Upon admission, new clients will be briefed on group home policies by staff and must sign a non-negotiable agreement acknowledging the rules.
Quarterly treatment team meetings will be held to monitor placements, with ongoing informal evaluations. Any significant client-related events will be communicated to the team as needed. Butterflyz LLC adheres to each youth’s Wrap Around Program Plan of Care to ensure appropriate support.
Background & Referral Process
Referring agencies must provide background information to assess placement suitability, including age, developmental level, disabilities, legal history, school attendance, behavioral concerns, and supervision needs. Supporting documents such as permanency and treatment plans, court orders, social histories, psychiatric assessments, and physical exam results (including TB test results) will also be required.
Before placement, Home Away From Home Living Center and BYAC caretakers, along with social work staff, will review each potential resident. Existing group home clients' safety and well-being will always take priority in referral decisions.
Upon request, a pre-placement visit can be arranged to determine suitability. New clients will receive an orientation on group home policies and rules from group home parents and their Case Manager, signing a non-negotiable agreement acknowledging their understanding.
The treatment team will conduct quarterly meetings to review treatment plans and placement appropriateness, with ongoing informal evaluations. Home Away From Home and BYAC Living Centers strictly follow each youth's Plan of Care as outlined by the Wrap Around Program.
